Appy Health Inc. is the Employee, Patient, and Healthcare Consumer (Member) choice technology solution. As the Members HIPAA compliant, single source technology, that allows the Member to own, store, and manage all areas of their physical, mental, and wellness needs, is delivered via the Appy Health website, www.appyhealth.com and the Appy Health mobile app. By using the Appy Health Inc. website(s) or the mobile app, you consent to the data practices described in this statement.

By using the Appy Health Inc. member choice platform via the Appy Health web portal, mobile app, or any digital tools to access your personal and health information. Appy Health only uses your camera to capture the front & back of your government issued identification and the front & back of your health insurance benefits card. These images are stored securely in your Appy Health Profile.

Appy Health Inc. does not share, sell, rent, or lease its customer lists to third parties.
